Clinical analysis of complications of laparoscopic cholecystectomy

Abstract
During the period from September, 1990 to September 1997, 2710 cases of laparoscopic cholecystectomy were performed at the department of surgery, Chung Ang university, Seoul, Korea. We have done the retrospective study about complications and it¡¯s management of laparoscopic cholecystectomy and reasons of conversion to open cholecystectomy. The main results were as follows. There were 130 cases of postoperative complications(4.8£¥) including bleeding., bile duct injury, wound infection., acute renal failure, pulmonary complication and 4 deaths. The strictured common hepatic duct was reconstructed with Roux-en-Y hepaticojejunostomy, but other complicated patients were treated with conservative therapy. Total 37 cases of complication were converted to open cholecystectomy because of bleeding,, multiple adhesions., bile duct injury and bowel injury. We think that the preoperative bile duct evaluation and the selection of a suitable patient for laparoscopic cholecystectomy are important to avoid of bile duct injuries. And it is also important to do laparoscopic cholecystectomy in no hurry and when the operative maneuver is anticipated difficult on viewing laparoscopic image, prompt conversion to open method is important to avoid of bile duct injuries.
